Service Eligibility & Scope
I understand First Call Roadside Services provides mobile disc-brake service for passenger cars, SUVs, vans and light-duty pickups, plus front disc-brake service only for eligible box trucks with a $25 labor upcharge. First Call does not service drum brakes, semis or heavy equipment. The service is limited to the work specifically confirmed for this appointment and is not a comprehensive vehicle inspection.
Authority to Authorize Work
I represent that I am the vehicle owner or otherwise have authority from the owner to request, authorize and pay for the service. I authorize First Call Roadside Services to perform the confirmed brake work at the service location I provide.
Price, Parts & Additional Work
I understand the price shown or later confirmed applies only to the stated job scope. Rotor installation is $25 per rotor when selected. Eligible box trucks add $25 labor. Same-day Express Service, when available, adds $25 labor. A missing wheel-lock key or known stripped/damaged lug issue adds the stated labor charge. Parts, taxes or other items not included in the displayed labor price will be quoted when applicable. No additional repair outside the confirmed scope will be performed without my authorization.
Customer-Supplied Parts
If I supply parts, I am responsible for their fitment, completeness, quality and condition. First Call is not responsible for defects, failures, manufacturer warranty issues or delays caused by incorrect, defective, used or incomplete customer-supplied parts. If work has already begun and customer-supplied parts cannot be installed, labor already performed may still be due. Customer-supplied parts are not warranted by First Call.
Pre-Existing, Hidden & Unsafe Conditions
I understand disassembly may reveal rust, corrosion, seized hardware, damaged wheel studs/lug nuts, damaged wheel-lock components, fluid leaks, worn calipers, suspension issues, prior improper repairs or other conditions that were not visible beforehand. First Call is not responsible for pre-existing or hidden defects or for failures unrelated to the authorized work. If a condition makes the requested repair unsafe or impractical, First Call may stop, decline or reschedule the work and will discuss any additional repair or charge with me before proceeding.
Mobile Service Location
I will provide a reasonably level, stable, legal and safe work area with enough access around the vehicle. I will disclose known hazards, wheel locks, modifications and other conditions that may affect service. First Call may decline or reschedule service because of unsafe surfaces, traffic exposure, inadequate access, severe weather or other conditions that create an unreasonable safety risk.
Work Area Cleanliness
First Call takes reasonable care to keep the service area orderly and to collect normal packaging, disposable materials and service-related debris generated by our work before departure. This does not include remediation of pre-existing oil, grease, brake-fluid or other stains/leaks, environmental contamination, or conditions caused by vehicle defects that existed before or outside the authorized service.
Vehicle Movement, Road Test & Documentation
I authorize First Call to move the vehicle a reasonable distance and, when appropriate, perform a limited road test to verify the repair. I also authorize reasonable photographs of the vehicle, parts and work area for service documentation, estimates, condition records and dispute prevention. This does not authorize use of my images or personal information for advertising without separate permission.
No Guarantee of Unrelated Conditions
I understand brake noise, warning lights, vibration, pulling or other symptoms can have more than one cause. Completing the authorized brake work does not guarantee correction of defects or symptoms caused by components outside the confirmed scope. First Call may recommend additional diagnosis or repair if another condition is discovered.
Risk & Liability
First Call Roadside Services will use reasonable care in performing the authorized work. To the fullest extent permitted by law, I agree First Call is not responsible for loss or damage caused by pre-existing conditions, hidden defects, inaccurate or incomplete information I provide, defective/customer-supplied parts, unsafe service conditions, or failures unrelated to the work First Call actually performed. Nothing in this agreement waives liability that cannot legally be waived.
Post-Service Responsibility
I agree to follow any post-service instructions provided. If I experience a brake warning light, fluid leak, severe or unusual noise, soft pedal, loss of braking performance, loose-wheel concern or other safety issue after service, I will stop driving as soon as safely possible and contact First Call promptly. I understand continued driving after a known safety warning can cause additional damage or risk.
Scheduling, Arrival Windows & Payment
I understand this form is an appointment request and does not guarantee a date or time until First Call confirms it. Two-hour windows are estimated arrival windows and may shift because of traffic, weather, parts availability, safety conditions or earlier jobs. Payment for authorized work is due upon completion unless First Call agrees otherwise in advance.
Electronic Records
I agree that my checked acknowledgments, typed initials, printed legal name and electronic signature are intended to document my authorization and consent. I understand the submitted request, pricing record, consent version and submission date/time may be retained as part of the service record.
Electronic service record includes the request details, pricing record, consent/privacy versions, initials, signature acknowledgment and submission date/time.
